The UK National Screening Committee recommends that screening of children’s eyes should be offered to all children at age 4 to 5 years. This service should be organised and led by orthoptists.

In this age group the condition detected is amblyopia - a condition where the eye appears normal but the vision is reduced. If left untreated amblyopia can be a permanent, lifelong visual deficit.

The screening enables a test i.e. Keeler Crowded logMAR vision test, to be delivered to a defined population i.e. children aged 4 to 5 years, to identify those at risk of the target condition i.e. reduced vision.

Evidence shows that the optimum age to screen is 4 to 5 years old. It is more cost effective to undertake the screening in school when it has the highest specificity and coverage compared to pre-school screening programmes.

In April 2015 the Welsh Government produced a Health Circular on the delivery of the school vision screening for Wales. It outlined the care a child should receive in terms of delivery of the screening, the onward referral of a child who fails, and the eye examination the child should receive.

Any screening programme should be monitored and audited to ensure the quality and integrity of the screening provided. The Welsh Government requires an annual report of the screening from each Health Board.

Dylai unrhyw raglen sgrinio gael ei monitro a'i harchwilio i sicrhau ansawdd a chywirdeb y sgrinio a ddarperir. Mae Llywodraeth Cymru yn gofyn am adroddiad blynyddol o'r sgrinio gan bob Bwrdd Iechyd.

The screening is carried out at school entry, Reception Year 0, when a child is between 4 to 5 years old. The screening runs during the school academic year September to July. It covers children in mainstream schools, those who are educated at home or in independent schools, and children in special schools.

The role of the vision screener is to carry out the Keeler crowded logMAR vision test correctly, in a safe environment, record and report the results accurately and instigate referral for a full eye examination if the child does meet the pass criteria.

The screening is parental opt-out. This means a child will have their vision tested unless a parent has chosen not to have their child screened. The parent will be provided with information about the screening and the consent policy, giving them the opportunity to decline.

The vision screeners liaise with a school to agree a date and time for the screening to take place, agree a suitable screening space and confirm the number of children to be screened. Ideally two vision screeners should be available for each screening session.

A letter advising the parent of the outcome of the screening is completed for each child at the time of the vision test and placed in a sealed envelope for the school to pass to the parent, or the letter is completed later and sent by post. For those children who require referral, the parent letter includes information on the need for further tests and the local pathway for the tests.

The results of the screening are entered onto the National Child Health database. This should include the vision score, if a child was unable to complete the test, if a child was not screened e.g. if absent, or no consent, if a child requires referral, or a repeat test is required.

Children who attend special school or have special educational needs may not be able to do the vision test and require a vision test more appropriate for their ability and developmental stage.

Local arrangements for confirming consent, liaising with schools, the communication letters sent to parents, referral route for those who fail and arrangements for children in special schools / special educational needs will vary. The Orthoptist lead in your area will advise you of the local arrangements.

A child with a vision of less than 0.200 logMAR on the Keeler crowded logMAR test in one or both eyes is referred for a full eye examination. This examination will include assessment of eye position and eye movements, the ability to use the eyes together (binocular function) and the focusing error of the eyes (refractive error) with dilating eye drops if required (cycloplegic refraction) and examination of eye health (fundus and media examination).

The parents of a child who requires referral will have been advised of the need for the more detailed eye test and the local pathway for the tests in the outcome of screening letter. The route for these tests may be to an optometrist or to the hospital eye service. The Orthoptist lead will advise you of the local referral pathway for the eye examination, diagnosis and treatment in your area.

If a child achieves a pass vision (0.200) in each eye, the parent receives a letter advising the screening does not suggest reduced vision. They are advised that children are entitled to free eye tests with an optometrist.

Eye test

The school vision screening at age 4 - 5 years is the only opportunity for every child to have a vision test. Amblyopia is the main problem found by the screening in this age group. The most common predisposing conditions for amblyopia are squint (eye turn) and focusing problems requiring glasses (refractive error).

Detection of the reduced vision enables treatment to be given at time when a child’s visual development is still taking place. If the presence of reduced vision is missed the child is at risk of a lifelong, permanent visual defect which could adversely affect their educational attainment, employment opportunities and have psycho-social implications.

The vision screening should find most visual defects that impact on vision but as with all types of screening it is not perfect and may not identify every child with a visual problem. The vision test assesses distance vision only, which is important, but it does not indicate how well the eyes work together or see for close work. It also does not give any information about the health of the eyes. The screening will identify children with undetected reduced distance vision but it can't be relied on to provide the same results as a comprehensive eye examination.

Integral to the delivery of the screening are internal risk management and quality assurance processes to ensure an effective screening programme. This should include mechanisms to audit the screening, report incidents, ensure staff training and development, and appropriate links with internal governance arrangements.

Current campaigns promoting eye health in Wales

Ymgyrchoedd presennol sy'n hybu iechyd y llygaid yng Nghymru

Eye health promotion has three components - health education, eye care services and advocacy.

Education

Eye health education campaigns are generally directed at improving public knowledge and changing attitude to bring about a change in behaviour and prevention of eye defects. For example:

  • The importance of a healthy diet and weight control (diabetic eye disease, age- related macular degeneration).
  • Wearing sunglasses that block out 99% to 100% of UV-A and UV-B radiation (cataract, age-related macular degeneration), and wearing goggles to prevent eye injuries.
  • The cessation of smoking (hypertension, stroke, thyroid eye disease).
  • The importance of regular eye health examinations by an Optometrist, by highlighting the risks of family history in glaucoma (damage to the optic nerve) and the association of eye health as an indicator of general disease (hypertension, diabetes, neurological problems).

Eye care services

Health education has to occur alongside improvement in services. Eye health services are provided in the primary care sector (optometrists) and secondary care (hospital eye services). Promotion of eye health can be achieved by making these services more accessible, effective and acceptable to the public. In 2013 the Welsh Government published ‘Together for Health, Care Delivery Plan’ with the aim to improve eye health in Wales and support those living with sight loss. The development of the plan is supported by policy strategies that include the development of the Eye Health Examination Wales scheme, the Health Circular on school vision screening, the Low Vision Wales Scheme and Diabetic Eye Screening Wales.

Advocacy

This involves activities that raise the profile of blindness and the implications of sight loss to gain political support for changes in services and prevention policies. This is a key driver for the eye health care professional bodies (Royal College of Ophthalmologists, British & Irish Orthoptic Society, Optometry Wales) and charitable organisations (RNIB, Guide Dogs).

Gwasanaethau gofal llygaid

Rhaid i addysg iechyd ddigwydd ochr yn ochr â gwella gwasanaethau. Darperir gwasanaethau iechyd llygaid yn y sector gofal sylfaenol (optometryddion) a gofal eilaidd (gwasanaethau llygaid ysbytai). Gellir hyrwyddo iechyd llygaid trwy sicrhau bod y gwasanaethau hyn yn fwy hygyrch, effeithiol a derbyniol i'r cyhoedd. Yn 2013 cyhoeddodd Llywodraeth Cymru ‘Law yn Llaw at Iechyd: Cynllun Cyflawni Cymru ar gyfer Gofal Iechyd Llygaid’ â’r nod o wella iechyd llygaid yng Nghymru a chefnogi’r rhai hynny sy’n byw â cholli golwg. Cefnogir datblygiad y cynllun gan strategaethau polisi sy'n cynnwys datblygu cynllun Archwiliad Iechyd Llygaid Cymru, y Cylchlythyr Iechyd ar sgrinio golwg mewn ysgolion, Cynllun Golwg Gwan Cymru a Sgrinio Llygaid Diabetig Cymru.

Mae hyn yn cynnwys gweithgareddau sy'n codi proffil dallineb a goblygiadau colli golwg i ennill cefnogaeth wleidyddol dros newidiadau mewn gwasanaethau a pholisïau atal.
